Regional Myocardial Function during Acute Coronary Artery Occlusion
نویسندگان
چکیده
Myocardial regional function during acute coronary artery occlusion was studied using ultrasonic dimension gauges in open-chest dogs. Three pairs of 2mm ultrasonic crystals were implanted 1 cm apart near the endocardium in an ischemic segment, a control segment, and a segment at the margin of the ischemic zone. In the ischemic segment, coronary artery occlusion resulted in prompt dyskinesis which progressed to holosystolic expansion; length at enddiastole (diastolic length) increased by 11%, segment stroke work decreased by 91%, and the diastolic pressure-length relationship was displaced and steepened. In the marginal segment, active shortening and stroke work decreased by 37% and diastolic length increased by 4%. In the control segment, an initial increase in active shortening was observed, followed by compensatory operation of the Frank-Starling mechanism. Nitroglycerin administered during coronary artery occlusion decreased diastolic length and increased shortening in all three segments. An early beneficial effect of isoproterenol on all segments was later replaced by deterioration in marginal and ischemic segments. After propranolol administration, the decrease in shortening of the marginal segment was reduced to half of that observed during a control coronary artery occlusion, suggesting a protective effect of this drug. These results indicate the power of this approach, which provides continuous quantification of regional wall function in myocardial ischemia and during therapeutic interventions.

Rapid Spontaneous Coronary Artery Reperfusion Following Severe Bradycardia During Exercise Tolerance Test: A Rare Case Report
The acute occlusion of a coronary artery without the evidence of myocardial infarction might immediately affect both the diagnosis and treatment of coronary artery disease. We present the case with a sudden occlusion of the left main coronary artery without typical chest pain following an exercise tolerance test and rapid spontaneous reperfusion of the left main coronary artery.

The immediate functional deficit resulting from acute regional myocardial ischemia was evaluated in 40 anesthetized dogs. Ventricular function curves, maximum dp/dt, isovohimetric force-velocity curves, and peak systolic and resting diastolic length-tension curves were assessed at fixed heart rate and constant aortic pressure before and during occlusion of the anterior descending coronary arter...

OBJECTIVES This study sought to assess the effects of sequential coronary artery occlusion during minimally invasive coronary artery bypass graft surgery (CABG) on hemodynamic variables and left ventricular systolic function by means of transesophageal echocardiography (TEE). BACKGROUND Clinical and experimental studies suggest a protective effect of ischemic preconditioning in patients with ...
متن کاملCorrelation of regional myocardial blood flow and function with myocardial infarct size during acute myocardial ischemia in the conscious pig.
Regional myocardial function and blood flow were determined for 48 hours after permanent occlusion of the left circumflex coronary artery in conscious swine. Systolic wall thickening and end-diastolic wall thickness (EDWTh) were correlated with regional myocardial flow (RMBF) at 15 minutes, 24 and 48 hours after occlusion. Both regional function and blood flow were compared with the extent of m...
